According to many dermatologists, there are four types of severe acne.

Severe acne affects more than just the physical body; it can emtionally and psychologically take over every aspect of a person’s life. One’s self-esteem is impacted and the person can be withdrawn from scociety.

Severe Acne #1: Acne Conglobata

Of the four sever acne listed here, Acne Conglobata is the most severe one.  Characteristics of Acne Conglobata are large and have great number of inter-connected nodules accompanied by blackheads.  These skin lesions can become ulcerated (open wound) thus leading to skin scarring. Acne Conglobata can be found on the face, back, chest, upper arms, thighs and buttocks (bum).

Acne Conglobata usually occurs between the ages of eighteen and thirty an occurs more often in males than in females.  Acne Conglobata is usually preceded by acne cysts, papules or pustuules that do not heal.

 Treatment for Acne Conglobata is Isotretinoin.

Severe Acne #2: Acne Fulminans

Acne Fulminans is the sudden onset of Acne Conglobata (See above).  It affects mainly young males.  It’s characteristics are similar to that of Acne Conglobata.  What makes it different is that the patient will experience fever, aching in the joints (kness and hips) and weight loss.

To help reduce inflammation, Corticosteroids or non-steroidal anti-inflammatory medications may be used.

Severe Acne #3: Gram-Negative Folliculitis

Gram-Negative Folliculitis is caused by imflamed follicles.  The inflamed follicles is caused by bacteria infection which in turn is  caused by long-term use of antibiotic treatment of acne.  The characteristics of Gram-Negative Follicultis are postules and cysts.

The name “gram-negative” is given to this type of severe acne is due to the fact that “gram” is a type of blue stain used in lab testing.  Bacteria that do not stain blue are called “gram-negative”. 

The reason that this form of acne is called “gram-negative” relates to the fact that gram is a type of blue stain used for laboratory testing for microscopic organism. Bacteria that do not stain blue are referred to as “gram-negative.”

The bacteria in Gram-Negative Folliculitis are resistant to many antibiotics. As such, Isotretinoin and antibiotics are used as treatment.

Severe Acne #4: Pyoderma Faciale

Pyoderma Faciale only affects female adults between the ages of twenty to forty.  Pyoderma Faciale characteristics are sudden appearance of large painful nodules, postules and sores on the face.  While Pyoderma Faciale only has a short lifetime of a year, it may have long-term impact with severe facial sacrring.
